Why Is My Elbow Swollen Months After a Fracture?

Some swelling after an elbow fracture is completely normal. Your body sends fluid to the area to protect and repair it. That’s normal. But if your elbow remains significantly swollen weeks or months after the injury, something else might be happening. Persistent swelling is worth paying attention to, and it’s worth getting evaluated.

Your Elbow Heals On Its Own Schedule

Fractures heal at different rates. Age, severity of fracture, and general health are all factors in how long it takes to heal. Some people have mild swelling for months, then recover completely without any further medical intervention. Others have continued swelling, which is a sign of a complication that needs to be addressed.

The key is paying attention to how the swelling behaves. Is it getting better slowly, staying the same, or getting worse? That pattern tells your doctor a lot about what’s happening inside the joint.

Common Reasons Your Elbow Is Still Swollen

Here are some commonly known causes of persistent swelling in your elbow months after a fracture:

1. Post-Traumatic Arthritis

A fracture that runs through the joint surface can damage the cartilage. That damage triggers inflammation over time. Post-traumatic arthritis often develops gradually after elbow injuries. You usually develop swelling, stiffness, and tenderness that get worse with activity.

  • Bursitis

The olecranon bursa sits just behind the point of the elbow. It can fill with fluid after an injury or prolonged pressure. Bursitis produces a soft, visible lump at the back of the elbow. It’s usually painless unless infected or irritated.

2. Heterotopic Ossification

This is a condition where bone forms in the soft tissue around a joint. It sounds unusual, but it’s more common after elbow fractures than most people realize. The new bone restricts movement and creates visible swelling. It typically develops within the first few months after injury.

3. Incomplete Healing or Malunion

Sometimes a fracture doesn’t heal in perfect alignment. A malunion places extra stress on the joint and surrounding tissue. That abnormal alignment can place ongoing stress on the joint and surrounding tissues, contributing to persistent inflammation and swelling.

4. Synovitis

The synovial lining inside the joint can become inflamed after trauma. Synovitis causes the joint to produce excess fluid. This fluid accumulates and makes the elbow appear swollen and feel tight.

5. Infection

Any swelling with warmth, redness, or fever needs immediate evaluation. Post-fracture infection can develop weeks or even months after injury. This is less common but identifiable with a proper evaluation.

When Should You Be Concerned?

See an orthopedic doctor if your swelling is accompanied by any of the following:

  • Increasing pain rather than improving pain
  • Significant loss of range of motion
  • Warmth or redness over the swollen area
  • A new lump or visible deformity
  • Fever or general illness

Even without these signs, swelling that hasn’t improved after three months deserves a closer look. Imaging studies like X-rays or an MRI can reveal what’s happening beneath the surface.

Treatment Options

Treatment depends entirely on the cause. Bursitis often responds well to rest, compression, and aspiration. Post-traumatic arthritis may be manageable with physical therapy, anti-inflammatory medications, or corticosteroid injections.

Heterotopic ossification sometimes requires surgical removal if it significantly limits movement. Your doctor will guide you based on your imaging findings and symptoms.
